Self-directed services have been growing. As participants and natural caregivers age, pathways for sustainable long-term caregiving need to be established to ensure continuation of services and supports that are cost-effective while fostering an individual’s self-determination. The presentation will highlight explorations in three states to document the needs, challenges and barriers of the I/DD community and discern resources needed for creating later-years transition plans and pathways. This is essential planning for waiver participants and administrators to tackle.
