University of Wisconsin – Madison will present about Supported Decision-Making for youth with special health care needs as a tool to encourage independence in decision-making. A Supported Decision-Making agreement can be used as an alternative to or as a tool under guardianship. Parent and attorney George Zaske will join Kristen as her guest.

Participants attending will be able to:
1. Describe the differences between Supported Decision-Making, shared decision making, and guardianship.
2. Recognize the role of the health professional in facilitating family decisions regarding tools to promote individual autonomy.
3. Discuss appropriate situations in which the health professional can use the different tools in supporting individuals with special healthcare needs.
4. Access resources that will support the use of Supported Decision-Making tools in their practice.
