By now everyone has probably heard that DHS has withdrawn the Family Care 2.0 proposal. The proposal was sent to the Joint Finance committee who did not take a vote. Below is press coverage from a variety of sources.

Since the Department of Health Services (DHS) submitted the Family Care/IRIS 2.0 Concept Paper to the Committee on Joint Finance on March 31st, committee members and others have asked a number of questions about why the Department chose to have three separate administrative zones if Family Care/IRIS 2.0 is implemented statewide. DHS has released a new webpage entitled "Family Care/IRIS 2.0: "Why Three?" which has additional information about the topic.

The Wisconsin Long Term Care Coalition sent a memo to members of the Joint Finance Committee asking them to delay voting on the Family Care and IRIS 2.0 concept paper until they receive additional information or clarification from the Department of Health Services.

The Wisconsin Department of Health Services has released a summary of the changes made to the Family Care/IRIS 2.0 Concept Paper based on feedback received from public hearings, testimony, and stakeholder meetings regarding the draft of the Concept Paper.

The Department of Health Services (DHS) has released a statement regarding its plans to develop a Division of Medicaid Services in hopes of streamlining programs such as Family Care and BadgerCare Plus.

The Wisconsin Dept. of Health Services (DHS) has submitted their concept paper for LTC Redesign ("Family Care/IRIS 2.0) to the Joint Finance Committee of the state legislature. Joint Finance now must consider the paper and vote to accept the redesign proposed by DHS or reject the proposal for continued changes. Joint Finance cannont alter the plan themselves.

The Wisconsin Dept. of Health Services sent out a news release today with some details about the proposed redesign of the long-term care system. The news release is in advance of the concept paper scheduled to be released the first week of March. The release includes information about Choice for Members and Quality Measures.

DHS has scheduled two public hearings that will be held March 7, shortly after the concept paper for the redesigned system is released. They will accept written comments until March 7th.

Survival Coalition Press Release:  Survival Coalition has released a side by side comparison of how the Joint Finance Committee’s budget motion would change Wisconsin’s long term care system compared to the Governor’s original proposal.

On Wednesday May 27th, the Joint Finance Committee voted 12-4 on a motion that pulls the original proposed long-term care changes from the state budget, but includes significant changes to Wisconsin's Family Care/IRIS programs that would begin in 2017.

Changes in the motion include:

  •  Expands Family Care/IRIS statewide by January 1, 2017.
  •  Changes the size of regions currently served by Managed Care Organizations (MCOs) so there are no less than 5 regions.
  •  Requires multiple “Integrated Health Agencies” (IHAs) in each region that can be for-profit insurance companies.
  •  Lifts the limits on profits and administrative costs on all entities providing Family Care.
  •  Ends IRIS as a separate Medicaid program, and requires IHAs to have a consumer-directed option that allows developing plans and allows people to select their own providers, thus folding the existing IRIS program into managed care oversight.
  •  Directs the Department of Health Services (DHS) to consult with stakeholders before final waiver request development, with no details on who those stakeholders would be or how they would be consulted.
  •  Retains Aging and Disability Resource Centers (ADRCs) and require DHS to evaluate the functional screen and options counseling reliability and consistency across ADRCs and require DHS to study ADRC governing boards and provide a report on proposed changes to the requirements of boards to remove duplication.

The motion also provides no assurances that elements of the existing waivers will remain unchanged. For example, there is no language saying service types or levels will remain the same.

Next Steps

The full Legislature still has to vote on the entire state budget.

Aging & Disabilities Professionals Association of Wisconsin Press Release: More than 35 County Boards across the state have passed resolutions requesting Wisconsin’s legislature take Aging & Disability Resource Centers (ADRCs) and Long-Term Care changes out of the proposed state budget.
